Submitting this completed form to irb@dordt.edu indicates that the investigator(s) has read and understood the IRB policies as published here and agrees to abide by the ethical practices indicated. The investigator should also understand that revisions to the proposed research may be required prior to IRB approval; approval of proposed research is not guaranteed.
For student investigators, faculty sponsors must send an email to irb@dordt.edu that confirms his/her knowledge and support of the proposed research; proposals will not be reviewed until the sponsorship email has been received.
